What you'll do

• Conduct initial assessments, admissions, case management, and follow-up visits
• reassess client needs and progress
• develop and update care plans based on physician orders and assessments
• facilitate referrals to services such as occupational and physical therapy
• perform medication set-ups
• coordinate and supervise CNA, PCA, and HHA visits
• report changes in client condition to physicians and the Director of Nursing
• maintain an active RN license.
