What you'll do

• Provide evidence-based assessment, intervention, consultation, and referral
• develop behavioral health, substance-use, and chronic disease care plans
• assist primary care providers with screening and treatment
• participate in quality improvement and integration strategies
• consult with and train healthcare team members
• complete timely documentation
• maintain community resource relationships.

Why this role

Master’s degree and North Carolina LCSW or LCMHC licensure in good standing (or equivalent) required; two years’ public health experience required. A $2,500 sign-on bonus is stated for fully licensed providers.
