What you'll do

• Triage patient calls and escalate to advanced practice providers or physicians as needed
• schedule audio visits and follow-up appointments
• support the Lung Health Initiative
• assess patients during nurse visits
• develop patient education on medications and disease states with providers
• manage medications, education, prior authorizations, and monitoring
• coordinate patient care, including evaluation of Endobronchial Valve candidates.
