What you'll do

• Manage the RAI process from resident admission to discharge
• complete MDS assessments accurately and on time
• develop individualized care plans
• oversee resident care data collection
• coordinate assessments and services across clinical departments
• support audit and survey preparation and response
• manage clinical authorization and justifications
• lead daily PPS/OBRA case management meetings and participate in quality assurance meetings
• support resident rights and staff development.
