The Care Coordinator, Substance Abuse Services is an onsite role in Chicago, IL, with a stated pay range of $50,000–$55,000 per year. The coordinator assesses patients’ psychosocial needs, connects them with appropriate services, and supports follow-up so they can access care and move to the next level of care. The role works with providers, case managers, social workers, and other departments to address barriers and coordinate substance abuse services.

What you'll do

• Conduct face-to-face substance abuse psychosocial assessments
• review and update assessments daily
• facilitate referrals consistent with assessed needs and physician recommendations
• meet patients for follow-up reviews or updates as needed
• provide brief interventions, stabilization services, and supportive counseling
• provide medication education printouts under program director direction
• complete and file documentation promptly.
