What you'll do
- Complete care plans
- track physician and ancillary visits and orders
- discuss resident observations and concerns with coordinators
- conduct wound rounds and coordinate wound care
- coach and direct staff as needed
- complete discharge planning and skilled nursing certifications and recertifications
- participate in weekly Medicare, therapy, and resident meetings
- act as therapy liaison
- complete reassessments and hospital visits when appropriate
- facilitate restorative programs
- participate in morning meetings
- assist with survey processes and doctor rounds
- serve as rotating on-call nurse in charge.
