What you'll do

• Conduct comprehensive patient assessments and develop individualized hospice care plans
• coordinate nursing care and symptom management with interdisciplinary team members
• educate and support patients and families on disease progression, care options, and advance directives
• monitor patient status and adjust plans as needed
• document interactions and clinical updates accurately and promptly
• connect patients and families with providers and community resources
• follow hospice policies, regulatory requirements, and confidentiality standards
• participate in team meetings, case conferences, and quality improvement initiatives.
