What you'll do

• Provide day-to-day clinical leadership and supervision for LVNs, Community Health Workers, and Care Coordinators
• oversee Enhanced Care Management and case management
• provide clinical oversight, care planning, risk stratification, and care-transition support for complex patients
• provide direct nursing care, assessments, education, and care coordination as needed
• lead staff onboarding and training
• monitor protocols, compliance, documentation, and quality improvement
• assist with staffing, scheduling, and clinical policies.
