What you'll do

• Provide clinical leadership across seven communities
• travel between communities based on clinical and operational needs
• conduct resident assessments and oversee individualized service plans
• monitor changes in resident condition and coordinate care
• guide caregivers and community leaders
• oversee medication management and clinical documentation
• coach and train care teams
• support communities facing staffing, leadership, or clinical challenges

• promote clinical standards, quality, safety, and regulatory compliance
