Compensation is $0 per year. The Referral Coordinator role is onsite in Jackson, TN. Christ Community Health Services is a faith-based Federally Qualified Health Center serving underserved communities with primary care, dental, behavioral health, pharmacy, and specialty services. The coordinator helps patients receive complete preventive and follow-up care by managing referrals, records, authorizations, and communication among patients, clinics, insurers, and outside providers. Accurate tracking and follow-through help keep care coordinated and medical records current.

What you'll do

• Route incoming faxes to the correct clinic and identify urgent documents
• follow up to ensure faxed information is processed
• review consult notes for status
• retrieve and transfer medical and dental records
• identify resources and schedule or facilitate external referrals
• obtain insurance authorizations when needed
• communicate with patients, hospitals, insurers, and providers
• follow up on incomplete referrals
• maintain referral tracking data and community resource directories
• and coordinate referral status with patients and clinical staff.
