The Registered Nurse (RN) – Home Health role is a field-based, onsite position in Crystal River, FL, serving patients in their homes within an assigned territory. Compensation is not specified. The nurse serves as the clinical lead for assigned patients, managing care from admission through discharge. Through assessment, care planning, skilled nursing, education, and coordination with the care team, the role aims to improve outcomes and reduce hospitalizations and readmissions.

What you'll do

• Conduct comprehensive in-home assessments
• develop and manage individualized care plans with physicians and care teams
• provide skilled nursing, medication management, treatments, and interventions
• coordinate care across therapists, aides, and physicians
• monitor progress and make clinical decisions
• educate patients and families
• coordinate transitions and discharge planning
• document in the EMR
• supervise and support Home Health Aides as needed
• participate in an on-call rotation as appropriate.
