The Registered Nurse (RN) Case Manager role is an onsite, full-time position in Brewster, MA, paying $55–$60 per hour. Cape Cod Post Acute and Healthcare Center is a 135-bed sub-acute and long-term care facility focused on compassionate, patient-centered care for residents. This role coordinates care across the healthcare continuum, supporting clinical decisions and continuity for residents with chronic, acute, or complex conditions. The case manager assesses needs, develops and evaluates individualized care plans with interdisciplinary teams, advocates for residents, and coordinates communication, appointments, referrals, and education for patients and families.

What you'll do

• Coordinate care for residents with chronic, acute, or complex conditions
• assess physical, psychosocial, and environmental needs
• develop and evaluate individualized care plans with interdisciplinary teams
• monitor progress and adjust plans
• advocate for resident preferences
• coordinate communication, appointments, referrals, transportation, and community resources
• support appropriate resource use
• educate patients and families.
