What you'll do

• Screen and assess adults
• develop and update individual service plans
• link clients to mental health and social services
• coordinate with jails, emergency rooms, hospitals, and community partners
• assist with housing, income support, transportation, and daily living services
• document services in the electronic health record
• track discharges
• participate in treatment team staffing, training, outreach, and consultation
• transport clients as needed.
