Case Manager — Youth Center

The Case Manager role is onsite in Woonsocket, RI, at a drop-in center serving youth ages 12–25. Compensation is not specified. Community Care Alliance provides services and supports intended to help community members address economic, social, and emotional challenges. The case manager supports youth participation and success in the Workforce Innovation and Opportunity Act program through trauma-informed services. The role combines direct case management, workforce and leadership programming, community coordination, and crisis response.

What you'll do

• Maintain client files and monitor program contract performance
• support recruitment, attendance, skill attainment, and job placement goals
• present youth resources to community partners
• design and facilitate educational and leadership programming
• provide intake, assessment, follow-up, and ongoing support
• participate in wraparound and family meetings
• advocate for youth
• support crisis intervention, safety screening, and connections to emergency services.
