What you'll do

• Arrange community-based services in collaboration with the RN care coordinator, clients, and families
• coordinate Medicaid application processes and emergency services as applicable
• develop and implement care plans
• complete assessments, follow-ups, reviews, and reassessments within stated timelines
• document care management activity and maintain confidential case records
• monitor service delivery and follow up on client needs
• authorize provider payments within applicable standards
• maintain knowledge of community resources and program requirements
• participate in interdisciplinary conferences.
