What you'll do

• Conduct initial and ongoing comprehensive assessments
• apply hospice admission and recertification criteria
• implement, develop, and document plans of care and recommend revisions
• educate patients and families about disease processes, self-care, end-of-life care, and nutrition
• determine service scope and frequency
• perform preventive and rehabilitative nursing procedures
• apply infection-control precautions
• provide clinical direction to hospice aides and LPNs/LVNs
• prepare clinical and progress notes
• communicate changes and outcomes to physicians and other personnel
• monitor cases for payer compliance
• and perform assigned on-call responsibilities.

Why this role

Requires a current registered nurse license and current driver’s license. Two years of clinical registered nursing experience and hospice experience are preferred; current CPR proof is required.
