Service Facilitator

What you'll do

• Coordinate and monitor Medicaid Home and Community-Based Waiver services
• conduct assessments and routine and reassessment visits
• develop and update person-centered Plans of Care
• monitor health, safety, and welfare and identify changes that may require services or interventions
• maintain accurate documentation and submit required reports
• collaborate with individuals, families, healthcare providers, and community resources.
