Registered Nurse Care Coordinator

The Registered Nurse Care Coordinator role is onsite in Fort Dodge, IA, in an office and clinic setting. Compensation is $65,000–$85,000 per year. The position supports coordinated care within an assigned Patient-Centered Medical Home (PCMH) care team. The coordinator helps patients receive timely, connected care by identifying care gaps, arranging follow-up and referrals, supporting chronic condition management, and maintaining accurate care plans and records. The role works across medical, dental, behavioral health, and administrative teams to improve access, quality measures, and health outcomes.

What you'll do

• Conduct daily care-team huddles
• review and route Epic Outside Events In-Basket messages
• coordinate follow-up after hospital discharges and emergency visits
• monitor referrals and document barriers to care
• identify high-risk patients and flag concerns
• facilitate interdepartmental referrals
• assist with patient care plans
• contact patients about health status, goals, and follow-up appointments
• conduct outreach for preventive screenings, immunizations, and chronic disease monitoring
• review upcoming appointments
• check medication accuracy and pharmacy data
• educate patients on MyChart
• gather patient-reported vitals using approved equipment
• maintain patient attribution lists
• document in the EHR and close coordination tasks.
