Home Health Registered Nurse – Personal and Skilled Care

The Home Health Registered Nurse role is onsite in North Ridgeville, OH, serving the Cleveland area, with pay of $40–$43 per hour. Morning and night shifts are available. BrightStar Care provides one-to-one home care and hospital-level care in patients’ homes. The nurse assesses patients, monitors their condition, provides skilled nursing interventions, and coordinates care with physicians and other providers. Patient and family education, accurate documentation, and infection-control practices support safe, continuous care at home.

What you'll do

• Perform in-home assessments and monitor vital signs and symptoms
• identify and escalate changes
• administer medications and provide wound, IV, catheter, and other skilled care
• follow physician orders and care plans
• coordinate with physicians and interdisciplinary teams
• educate patients and caregivers
• document in the EMR
• follow regulatory, HIPAA, safety, and infection-control standards
• respond to urgent situations and coordinate hospital transfers when necessary.
