What you'll do

• Accept and manage patients within your designated geographical area of coverage.
• Plan, prepare, and implement customized therapy programs designed to restore overall function and promote independence in the home setting.
• Develop and maintain individualized care plans/goals in collaboration with physicians, patients, and their families.
• Ensure timely and accurate documentation of all patient visits and therapy services.
• Effectively communicate discharge planning with patients, families, and interdisciplinary team members.
• Maintain compliance with all clinical and documentation standards.
