What you'll do

• Diagnose, manage, and treat acute and chronic conditions
• address social determinants of health
• conduct wound care, post-hospital transitional care, and condition-specific assessments
• complete medication reconciliation, fall-risk assessments, advance care planning, and health education
• coordinate post-discharge visits within 72 hours
• collaborate with care teams to support continuity and reduce readmissions.
