What you'll do

• Provide a minimum of four to six direct-service sessions per client per month as planned with the rehabilitation team and client
• develop and implement an Individual Rehabilitation Plan with clients every three to six months
• help clients resolve problems and access resources
• teach independence and self-advocacy
• provide crisis intervention as necessary
• maintain client charts and service documentation consistent with agency policies and COMAR regulations
• attend meetings and trainings
• research and provide client resources
• participate in agency-led groups or events at least monthly
• notify a supervisor of crisis situations.
