Registered Nurse (RN) Case Manager

What you'll do

• Perform comprehensive assessments and maintain individualized care plans

• provide health education and family support
• deliver hands-on nursing care including vitals, MARS/TARS, wound care, glucose checks, medication setup, and discharge planning.

Why this role

Requires an unrestricted Minnesota RN license and BLS certification and CPR. At least one year of nursing experience is required; home health or hospice experience is preferred. OASIS experience is preferred but not mandatory.
