What you'll do
- Provide in-person and telephone medical case management
- discuss treatment plans and alternatives with physicians
- assess, plan, implement, and evaluate patient progress
- evaluate treatment for appropriateness, medical necessity, and cost effectiveness
- attend provider, home, and sometimes attorney visits
- participate in discharge planning
- coordinate durable medical equipment and nursing services
- travel locally up to approximately 60% of the work week or month as needs require.

Why this role
