What you'll do

• Perform initial nursing assessments for new admissions. Coordinate and revise care plans with appropriate staff
• communicate patient progress to the clinical manager, physician, and care team
• maintain accurate patient records
• document pain and symptom-management outcomes
• educate families
• make referrals to hospice disciplines as needed or document the rationale for not doing so
• perform assigned on-call responsibilities.
