The supervisor oversees social work, participant advocacy, and behavioral health services at the PACE Center, ensuring participant-centered support and coordination with the interdisciplinary team. The role also manages team operations and helps address complex participant needs and concerns.

What you'll do

• Lead and supervise social work, participant advocacy, and behavioral health teams
• coach and manage direct reports
• oversee advocacy and enrollment
• serve as an escalation point for complex concerns
• partner with the interdisciplinary team on assessments and care planning
• coordinate staffing and assignments
• maintain community and agency relationships
• review participants at risk for disenrollment and coordinate interventions.
