Home Hemodialysis Care Partner — 1:1 Client

The Home Hemodialysis Care Partner role is onsite in Weedsport, NY, for three days a week; compensation is $30 per hour. Herewith Caregivers provides one-on-one support to a patient transitioning from clinic dialysis to home treatment, with training and clinical team guidance. The care partner helps maintain a safe, organized treatment environment and supports the patient’s home dialysis routine. The role contributes to the patient’s comfort and stability through treatment setup assistance, companionship, light daily-living support, safety monitoring, and communication with the coordinating nurse.

What you'll do

• Assist with setup for NxStage or Tablo dialysis treatments
• keep the treatment environment clean, organized, and safe
• provide companionship and reassurance
• help the patient stay on treatment schedule
• provide light activities-of-daily-living support as needed
• monitor safety and report concerns to the coordinating nurse
• maintain simple documentation and shift notes.
