Registered Nurse (RN) Case Manager, Hospice

What you'll do

• Provide nursing services according to the plan of care
• assess and reassess patient and family needs
• document goals, interventions, and responses
• collaborate with physicians and the interdisciplinary team
• instruct patients and families in self-care when appropriate
• supervise and evaluate aides
• complete supervisory visits and clinical notes
• participate in the on-call rotation as needed.
