Case Manager – Full Circle Program

The Case Manager – Full Circle Program role is onsite in Rapid City, SD. Compensation is not specified. The Case Manager helps clients meet case management needs and recovery goals during and after treatment. Through life-skills groups, individualized planning, progress reviews, resource coordination, and documentation, the role supports clients’ recovery and connection to community services.

What you'll do

• Lead life-skills groups
• conduct individual intake sessions
• develop and implement person-centered case management plans
• assess and track progress
• educate clients about community resources
• facilitate aftercare planning
• locate support groups
• maintain community resource lists
• support a safe recovery-based environment
• coordinate client care with the team
• complete documentation
• attend training
• comply with ethics, policies, and HIPAA requirements
• may transport clients.
