What you'll do

• Provide RN clinical coordination for high-risk patients
• support assessment, triage, care planning, medication management, and patient education
• partner with clinical, operational, quality, finance, IT, patient access, and case management teams
• build partnerships with community organizations and referral sources
• track grant-funded program deliverables, outcomes, and compliance
• use data and continuous improvement to address barriers and improve service delivery.
