Targeted Services Coordinator

The Targeted Services Coordinator role is onsite in Rigby, ID, with compensation of $22–$24 per hour. The coordinator supports participants and their service teams through planning, monitoring, and documentation. The role exists to help ensure participant service plans are comprehensive, implemented, and responsive to individual needs and choices. It also contributes to quality assurance through accurate records, compliance reviews, and communication with participants, providers, guardians, and team members.

What you'll do

• Coordinate participant planning meetings
• develop comprehensive service plans
• meet monthly with participants, providers, and guardians
• monitor implementation and follow up as needed
• assist with plan changes
• ensure participants understand their choice of service providers
• review provider status reviews and follow through
• report suspected mistreatment, abuse, neglect, exploitation, or injuries of unknown origin to designated entities
• maintain working relationships with participant teams
• document services accurately
• maintain 90% compliance during quality reviews
• submit billing within a reasonable time
• assist the Administrator with quality assurance.
