Part-Time Service Coordinator

The Part-Time Service Coordinator role is hybrid in Chicago, IL, supporting Village Center and Swartzberg House. Pay is $21.63 per hour. CJE SeniorLife supports older adults in living with dignity, purpose, and connection. The coordinator assesses residents’ needs, connects them with services, and advocates for appropriate support. By coordinating with residents, families, housing management, and community providers, the role helps residents access resources that support independence and well-being.

What you'll do

• Assess new tenants’ activities of daily living and interests within their first two weeks
• arrange services such as homemaking, transportation, meals, and personal care
• conduct annual reassessments
• document and report changes in residents’ functioning
• work with residents and families
• coordinate with discharge planners and rehabilitation programs
• provide information and referrals to government-supported services
• work with providers on socialization, education, and health events
• advocate for residents when needed.

Why this role
