What you'll do

• Review schedules and referrals and note changes in residents’ conditions. Conduct brief, focused therapy sessions
• complete initial assessments and update treatment plans. Collaborate with nurses, rehabilitation staff, and providers about care needs and behavioral concerns. Document sessions and plans for compliance and billing, connect with family members when needed, and plan follow-ups.
