Job overview

The Case Planner – TFBH Program role is hybrid and in the field in Wading River, NY, with a salary range of $53,000–$55,000 per year. The case planner provides casework, group work, and advocacy to support children’s development and permanency planning. The role coordinates services, works with foster parents and families, and handles case information and documentation to support children’s needs and family relationships.

What you'll do

• Gather child and family social and medical history and provide case planning
• advocate for and obtain educational, clinical, and recreational services
• meet with foster parents, provide or obtain support, and monitor care
• engage families in casework and group work aimed at sustaining relationships and, when possible, reunification
• assist with adoption-related activities when necessary
• arrange foster parent training
• share emergency cell phone coverage and respond to after-hours or weekend emergencies.
