The Homeless Outreach Mental Health Practitioner role is onsite at RADIAS Health’s downtown office in Saint Paul, Minnesota. The starting salary range is $59,773–$62,987 per year, plus a $180 monthly office benefit package for parking or remote expenses, subject to change. The role supports the PATH Homeless Services Team and people experiencing homelessness who have mental-health or co-occurring needs.

What you'll do

• Provide outreach, screening, case management, and re-engagement services
• develop and revise recovery and housing plans with clients
• maintain a caseload of 15–20
• coordinate with a multidisciplinary team and community agencies
• conduct assessments
• support crisis intervention and referrals
• and complete required documentation.

Why this role

Requires 6,000 hours of supervised experience delivering services to people with mental illness and experience supporting people experiencing homelessness. The schedule is Monday–Friday, 8 a.m.–4 p.m. or 9 a.m.–5 p.m.; coordinated outreach may occur early in the morning or evening.
