What you'll do

• Assist with planning, organizing, implementing, evaluating, and directing Social Services Department operations
• supervise, coordinate, and perform department activities
• assess and address patients’ social, emotional, and psychological needs
• collect data and complete required documentation
• coordinate discharge planning and referrals
• maintain relationships with families and residents
• coordinate care plan meetings
• communicate with physicians, nursing staff, admissions, and other team members about care and transitions.
