What you'll do

• Conduct virtual outreach to identify, educate, and recruit people who may benefit from PrEP and HIV prevention services
• navigate patients to care, including appointment scheduling and clinical coordination
• provide individualized education on PrEP, nPEP, HIV prevention, medication adherence, and safer sex
• support retention and re-engagement
• use motivational interviewing and goal setting
• refer patients to primary care, behavioral health, and community resources
• document interactions in the EHR
• manage caseloads and outreach
• participate in data reporting, quality improvement, and team training.
