What you'll do

• Assess clients and families

• provide prescribed medications and treatments
• educate clients and families
• coordinate assigned caseloads, referrals, and discharge planning
• communicate with physicians and community providers
• complete clinical documentation
• supervise and evaluate home health aide care every two weeks
• organize travel and document mileage.
