What you'll do

• Assess patients and families to determine hospice needs
• provide nursing care and develop, initiate, and revise individualized plans of care
• administer prescribed medications and treatments
• counsel and educate patients and families
• coordinate assigned caseloads when acting as case manager
• maintain clinical notes and communicate changes to physicians and community health partners
• attend interdisciplinary meetings
• participate in on-call duties and be available every other weekend.
