What you'll do

• Meet regularly with students and families at home, school, or community locations
• maintain communication between meetings
• complete intakes, discharges, screenings, assessments, and action plans
• connect families with community resources
• review progress toward goals
• coach and mentor students and families
• participate in supervision and training
• coordinate program referrals with school personnel
• facilitate youth and family psychoeducational groups and parent education
• assist with program reports.
