What you'll do

  • Support pre-admission, admission, discharge, and follow-up processes
  • conduct facility tours
  • coordinate with the Liaison on patient acceptance
  • track referrals and anticipated admissions
  • arrange admissions
  • obtain prior approval numbers when authorized
  • ensure level-of-care approvals and required screens
  • prepare and distribute face sheets and clinical screens
  • verify Medicaid eligibility when appropriate
  • track hospital admissions and readmissions
  • coordinate weekend and holiday admissions with Liaisons
  • maintain resident, waiting, and inquiry lists
  • coordinate clinical assessments when the Liaison is absent.
