What you'll do

• Perform OASIS assessments and maintain accurate documentation.
• Provide skilled nursing care, treatments, and medication administration.
• Educate patients and families on care and disease management.
• Coordinate care with physicians and interdisciplinary teams.
• Support discharge planning and ongoing patient needs.
