The Care Coordinator — Behavioral Health role is onsite at the Peoria Clinic in the Denver Metro area, Colorado. Pay is $20.67–$24.03 per hour. STRIDE Community Health Center provides primary care, dental, pharmacy, behavioral health, health education, and outreach services to individuals and families. The coordinator supports quality and value-based care by connecting patients with internal and external services, managing referrals, and providing basic health education. The role also contributes to population-health outreach and helps patients and care teams follow through on identified needs and goals.
