What you'll do

• develop and update individualized care plans
• coordinate physicians, home care, specialists, and community resources
• manage transitions between hospital, rehabilitation, and home
• advocate at appointments and care conferences
• serve as primary contact for clients and respond to urgent needs outside standard business hours
• accompany clients to medical appointments and care-related visits as needed.
