Hospice Registered Nurse Case Manager

What you'll do

• Assess and reassess hospice patients’ nursing needs
• provide dietary counseling, nursing services, treatments, and preventive procedures
• report changes to physicians and interdisciplinary group members
• teach and counsel patients and families
• supervise and train nursing personnel
• develop and reevaluate care plans
• attend team conferences
• maintain records and confidentiality
• participate in the on-call system
• coordinate care for patients in facilities.

Why this role
