What you'll do

• Coordinate care and connect patients and families with providers, specialists, behavioral health services, and community resources
• develop and update individualized care plans
• provide individual and group education
• identify high-risk patients and monitor progress
• assess service utilization and care gaps
• collaborate with care teams
• maintain accurate documentation and timely follow-up in accordance with policies and requirements.

Why this role
