Job overview

The Physician role is onsite in Sacramento, CA; compensation is not specified. The physician provides care to InnovAge’s enrolled geriatric participants across its Day Center clinic and other care settings. InnovAge’s PACE programs coordinate care for enrolled participants. The physician evaluates and manages medical conditions, coordinates with specialists and hospital teams, and contributes to interdisciplinary care planning and quality improvement. The role supports continuity and quality of care through clinical leadership, documentation, and participation in program development.

What you'll do

• Evaluate, diagnose, and manage participant medical conditions
• take histories, perform examinations, order and interpret diagnostic tests, develop treatment plans, prescribe, and provide follow-up
• document care in medical records
• provide rotational after-hours phone on-call coverage every few weeks
• participate in interdisciplinary care planning
• contribute to quality improvement
• provide input on medical programs and processes
• support department teamwork and culture.

Why this role

Monday–Friday, 8 a.m.–5 p.m.; four days of participant visits and one administrative day. Rotational after-hours phone call coverage occurs every few weeks; calls are first triaged by an RN.
