What you'll do

• Provide skilled physical therapy to help patients maintain, restore, and improve movement and function
• deliver interventions and modify them as needed
• conduct reexaminations at least every 30 days
• perform discharge planning
• collaborate with patients, caregivers, and the interdisciplinary care team.
