What you'll do

• Provide direct support and personal care
• promote self-help and social skills
• administer medication if certified and monitor health needs
• implement support plans and document progress in the electronic health record
• support community activities and relationships
• drive agency vehicles and maintain logs
• complete required training and maintain certifications
• provide coverage at alternate program sites as needed.
