What you'll do

• Coordinate and deliver high quality, patient-centered care.
• Assess patient condition, identify and report any changes.
• Perform procedures, monitoring, or other functions as ordered.
• Document administration of care.
• Perform administration of prescribed medications.
• Monitor patient for therapeutic response.
• Respond promptly to patient requests.
• Teach patients, families and caregivers about medical condition, treatment plan, medications and follow-up measures.
